*Only complete the section below if your conference fee is NOT already included in your membership or you are not a member of WI HSEA.
*If you paid the $125 membership, you do not need to pay for the conference.
Use the form on the left to request an invoice to pay by check or complete the Credit Card Payment form on the right to pay with a credit card.
